MEMBERSHIP IS NOT VALID UNLESS BOTH THE FORM AND PAYMENT HAVE BEEN RECEIVED BY THE OFFICE. THE OFFICE WILL CONFIRM BY EMAIL THAT YOU ARE A MEMBER OF UKPH, MEMBERSHIP IS AT THE DISCRETION OF THE BOARD.
